Abstract

In this article, both common problems with other European countries and specific problems in Spain are considered in order to propose appropriate lines of action and some specific changes that contribute to drawing a more exciting scenario for our National Health System (“The princess of the Welfare State”). Among the common challenges, the epidemiological and demographic challenge and the stagnation of apparent productivity stand out; and, among the specific ones, the rigidness of labor relations, the wastage of formulas to make management more flexible, the lack of competent governance of public-private collaboration, the deterioration of Primary Healthcare and the risk of evolving towards a two-tier health care system, are particularly relevant.It shows that reillusioning the princess of the Welfare State is not going to be an easy task, it is going to require new narratives, consensus and alliances and a large dose of good governance (impartiality, regulatory quality, government effectiveness, corruption control, respect for the law), strengthen Primary Care, as well as recover clinical and health management based on trust in professionalism and within frameworks that promote care integration and the creation of value for individuals and populations.
